We're still buzzing from the incredible success of our recent Cadaver Lab Masteclass at the esteemed Nicholson Center in Orlando, USA.

We first thought of performing a geometric compensation mammoplasty, but she had a tuberous breast and the lower triangle for compensation was much smaller than the defect. Moreover, the final breast volume would be smaller than desired by the patient. So, we changed from a volume displacement technique to a volume replacement technique. The planning was modified to an advancement V-Y thoracolateral flap. We were able to correct the defect, the ptosis, and keep the same previous breast volume.

Skin-reducing mastectomy is often used for ptotic breasts, but is associated with a 25% rate of relevant complications in Maurizio Nava's report. The skin has an incredible capacity of retraction and skin-reducing mastectomy may be avoided in many cases. I show here an example of a nipple-sparing mastectomy with immediate prepectoral implant-based reconstruction, in the case of a big breast with severe ptosis. I show here our step-by-step on how to manage a good result and avoid complications.

The extended V-Y Latissimus Dorsi flap is a useful tool for closing large chest wall defects. Instead of rotating, the myocutaneous flap is advanced. The lateral part of the flap closes the anterior wall and the medial part helps to close the back, in a V-Y shape. This V-Y design allows for a much larger flap than the conventional one.

The VRAM (vertical rectus abdominis myocutaneous) flap may close large chest wall defects with less morbidity, in a faster and safer way, although less aesthetic, than the TRAM flap.
